OSHA Code 1910.140(d)(2(ii)): "Personal fall arrest systems must be rigged such that an employee cannot free fall more than 6 feet (1.8 m) or contact a lower level." 

Unprotected Roof Edges

OSHA Code 1910.28(b)(13): “When work is performed less than 6 feet (1.6 m) from the roof edge, the employer must ensure each employee is protected from falling."

Unprotected Skylights

OSHA Code 1910.28(b)(3)(i): "Each employee must be protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level."

Unprotected Roof Hatches

OSHA Code 1910.28(b)(3)(v): "Each employee is protected from falling through a hatchway and chute-floor hole by a hinged floor-hole cover or fixed guardrail system."

Unprotected Fixed Ladders

OSHA Code 1910.28(b)(3)(iv): “Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toe boards erected on all exposed sides."

Unprotected Extension Ladders

OSHA 1918.24(c): “Where portable straight ladders are used, they shall be of sufficient length to extend three feet (.91 m) above the upper landing surface and be positively secured.” 

Unprotected Walking-Working Surfaces

OSHA 1910.22(c): “The employer must provide, and ensure each employee uses a safe means of access and egress to and from walking-working surfaces.”
